Individualized Healthcare Plan & Ask Rx
The Individualized Healthcare Plan (IHP) is developed in collaboration with the school, medical personnel, and family to address skilled medical needs of a child while in the education setting. This presentation will include information regarding Extended School Year planning for the eligible child and Answers4Families’ “Ask Rx,” an online resource for asking a pharmacist about the medications prescribed to your child.
