Immunization Information for Nebraska's 7th Graders Memo

Dear Parents of 6th Grade Students:

To enter 7th grade, your child must have the following:

A current physical (after January)
3 doses of the DPT vaccine, one given after the 4th birthday
2 doses of the MMR vaccine
3 doses of the Polio vaccine
3 doses of the Hepatitis B Vaccine (Please check if your child has this done, many do not)
1 dose of Varicella (chicken pox) vaccine (2 doses after 13 years old) OR documentation of the disease

When your child has been vaccinated, please send me a note or a copy of his/her immunization card so I can add it to their record at the school.

This is also a good time to take other children in your family for the same immunizations.
