For a dual eligible individual (eligible for both Medicare and Medicaid) insert the name of the medication. The spelling must be accurate. The last column "Covered for Duals" indicates if Medicaid will cover that drug for a dual eligible. The other columns give information about Medicaid limitations on coverage of that drug.
