Long-Term Case Management in Western Nebraska

The Aging Office of Western Nebraska (AOWN) Care Management Program is designed to provide identification, coordination, and facilitation of community services to assist elderly remain in their own homes. This program receives its certification from the Nebraska Health and Human Services, Division on Aging and Disability Services.

Case Managers are Registered Nurses or Certified Social Workers. They conduct a complete assessment of each individual to identify needs, develop and recommend a plan of care, help arrange for appropriate community resources, and provide regular follow-up to assure satisfaction and to identify changing needs. This program does not provide skilled nursing services.

The Long-term Case Management (CMU), by state statute, must determine the individual's ability to cost share in its provision. Each person's monthly income is applied to a sliding fee scale to assess their share. But no individual will be denied service due to their ability to pay.

The program is available to all older residents in the Panhandle of Nebraska. Referrals are received from any source. If you would like further information on this program, contact the Case Management Supervisor at the AOWN (Aging Office of Western Nebraska).

Supportive Services:
An array of other supportive services are also available from the AOWN either directly or by other providers contracted with the aging office.

Handyman/Chore:
In-Home assistance in the form of housekeeping and homemakers, yard mowing, snow removal, and minor repairs and renovation is available in limited areas of the Panhandle. Please call the AOWN for the available providers.

Information and Assistance:
Extensive identification of community services is available. This service will put you in contact with a service provision to assist in your particular need.

Legal Referral:
Legal assistance is available to those who need council. The AOWN will assist in the referral to appropriate service providers.

Others:
Telephoning, friendly visitor, and coordination with other service providers is also available. The AOWN staff is also available for speaking and educational presentations on aging services.

Health Promotion:
Promotion of good health and practices of wellness of the older population is accomplished by presentations, information and demonstrations by the AOWN. The AOWN is available to present to any organization or group, information on health practices and awareness for the elderly.

Long Term Care Ombudsman:

The Nebraska Department on Aging and Disability Services is designated to serve as the Ombudsman provider in the State. This program is the advocate for the well being of residents who are residing in long term care facilities. The AOWN will assist in the referral to the Department of any complaint, concern or issue affecting a nursing home resident.
