- Care Management
Care Management coordinates and arranges for services, which allow seniors to live independently for as long as possible. Care managers arrange for services, including transportation, housekeeping, home-delivered meals and emergency response systems.
- Senior Care Options (SCO)
Senior Care Options is available to persons age 65 or older who are considering admission to a nursing home, may be eligible for Medicaid and requests Medicaid payment for the nursing home care. Health and Human Services staff determines financial eligibility and SCO staff evaluates the need for nursing home care. If other options such as in-home care are viable, SCO assists in arranging those services.
- Medicaid Waiver
The Agency contracts with Health and Human Services to provide home and community-based services to persons age 65 or older. Persons who are eligible for Medicaid and meet nursing home criteria, but would rather stay in their own home, may be eligible for the Medicaid Waiver program. In-home services may include housekeeping, meals, personal care, adult day services and essential transportation. Assisted living in residential care facilities may be an option.
- Senior Health Insurance Information Program (SHIIP)
The SHIIP provides assistance to older persons regarding health insurance issues. The program trains volunteers to provide individuals assistance and information on: Medicare, Medicaid, Medicare supplemental insurance, Medicare-approved drug discount cards and Long-term care insurance. The SHIIP program is a “hands on” program, beneficial to a wide variety of individuals.
- Legal
The Agency contracts with local attorneys to provide legal advice in areas where an attorney’s expertise is required. Emphasis is placed on areas of law related to persons over age 60. The program assists individuals with simple wills, powers of attorney, advance directives (living wills and powers of attorney for health care), Medicaid and consumer issues. The trained staff person has knowledge of programs through human services agencies and assists in the understanding and needs of these programs. The staff also is available to speak on a variety of timely aging issues such as advance directives, financial counseling, consumer protection and long-term care insurance.
- Nutritional/Senior Centers
Nutritional services primarily consist of congregate meal programs at senior centers. The Agency contracts with 18 full centers and 14 meals-only sites, all of which offer home-delivered meals. By contracting with the centers and sites, local control allows the senior population to adapt services to local needs. The Agency contracts with a licensed nutritionist who helps with and approves menus. The Senior Center coordinator provides and arranges health and wellness related education.
