Aged & Disabled Medicaid Waiver
Home and community-based waiver services offer eligible persons a choice between entering a Nursing Facility (NF) or receiving necessary supportive services in the least restrictive and most home-like setting.
To be eligible for support through this “Aged and Disabled Waiver,” a potential client must meet the following general criteria:
Eligibility Criteria
- Have care needs equal to those of Medicaid-funded residents in Nursing Facilities;
- Be eligible for Medicaid;
- Physically reside in Nebraska; and
- Work with a services coordinator to develop an outcome-based, cost effective Plan of Services and Supports.
Eligibility Process
- Services Coordinator determines that the client meets the nursing facility level of care.
- Medicaid Eligibility Worker determines client's Medicaid financial eligibility
- Services Coordinator conducts assessment of client needs.
- Services Coordinator determines services to be provided and the amount of services to be authorized.
- Services Coordinator offers client choice of providers.
These services include assistance with or provision of personal care activities, activities of daily living, instrumental activities of daily living, and health maintenance provided in a licensed and waiver-certified facility.
The need for this service must be identified in one or more assessment areas and be addressed in the client's Plan of Services and Supports. The need for assisted living service is jointly determined and reassessed, as needs change by the client and services coordinator.
Eligibility Priority
- Transition/Transfer (clients already on the waiver who are moving from one geographic area or transitioning from children's waiver service to adult services or from receiving services as an adult with disabilities to the aged population)
- NF Transitions (current Medicaid-paid NF residents [not short-term, temporary, or rehabilitation basis] who want to move out of the nursing facility to in-home or AL services).
- Persons evaluated through the Senior Care Options program or adults with physical disabilities who have definite plans to move toa NF at Medicaid expense.
